What You'll Be Doing

Individualized Care Planning

Conduct comprehensive assessments of residents’ cognitive abilities, functional status, and personal preferences to develop individualized care plans. Collaborate with healthcare professionals, including physicians, therapists, and social workers, to create holistic strategies that address residents’ physical, emotional, and social needs. Adjust treatments as needed based on ongoing assessments and changes in residents’ conditions, ensuring personalized and responsive care.

Assistance with Activities of Daily Living (ADLs)

Provide assistance with activities of daily living, such as bathing, dressing, grooming, toileting, and eating, to support residents’ independence and dignity. Implement techniques and strategies to promote active participation in daily tasks, taking into account residents’ cognitive abilities and sensory preferences. Monitor residents’ performance with routine activities and provide appropriate support and encouragement, adapting care approaches as needed to optimize residents’ functional abilities.

Behavioral Management and Communication

Utilize specialized techniques and approaches to manage challenging behaviors and promote a calm and supportive environment. Foster effective communication with residents through verbal and non-verbal cues, active listening, and empathetic responses, recognizing and responding to residents’ emotions and unmet needs. Educate family members and caregivers on effective communication strategies and behavioral management techniques to enhance interactions with their loved ones in memory care.

Engagement and Therapeutic Activities

Plan and facilitate therapeutic activities and programming tailored to residents with cognitive impairments. Create a structured daily routine that includes meaningful pursuits and opportunities for socialization, promoting cognitive stimulation, emotional well-being, and a sense of purpose. Collaborate with team members, volunteers, and family members to design and implement engaging and person-centered programs that enhance residents’ quality of life and overall functioning.
