Director of Health and Wellness (RN or LPN)- Assisted Living and Memory Care

Directly oversee the delivery of care to Assisted Living residents to ensure that emotional, physical, psychological and safety needs are met through all program services and activities. Coordinate, along with the Executive Director, family/resident care conferences and serve as a direct liaison between residents, families, and staff.  Work with residents, families, and other staff to ensure that residents are at appropriate levels of care.  Manage labor costs, supplies, and other line items in clinical budget.  Monitor documentation systems to ensure that all regulatory timeframes for review of assessments and service plans are met and that resident needs are met. Write and review residents’ assessments/evaluations and care-plans with appropriate staff. Assess/evaluate potential residents for Assisted Living level of care.  This may involve assessing/evaluating potential residents in their home or another healthcare setting; must be able to drive to location and perform assessment/evaluation professionally and timely if necessary. Schedule and conduct new resident orientation for all Assisted Living Residents.  Provide, plan, schedule, and monitor staff training programs and in-services for the Assisted Living staff including statutory or regulatory required training. Attend all required training, in-service, and staff meetings. Manage and coordinate the work of all nursing and other assisted living staff, communicating pertinent resident health and safety information.  This includes overseeing and/or completing staff schedules monthly and daily assignments.  Strive to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices.  Ensure compliance with all state regulatory requirements including mandatory reporting to State. Ensure that all concern and incident reports are completed, filed and that follow-up is conducted appropriately.  Assist and/or perform in mock surveys.  Maintain a positive and professional demeanor toward residents, visitors, families, and co-workers while adhering to all policies, standards and procedures of Company. Require 24/7 on-call clinical support. Perform other duties as assigned.

Licensed Practical Nurse (LPN)

Responsible for medication administration for the residents in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines. May be responsible for the admissions of new residents to include all admission assessments and documentation. Responsible for completing acute and long-term care plans or service plans. May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the director. Provide general consultation regarding health-related concerns within the community. Notify physicians and/or family members of any change in resident’s health and provides proper documentation. Respond quickly to all emergencies. Maintain close communication with all departments and staff members. Provide educational meetings for residents and staff. Maintain confidentiality of residents’ and prospective residents’ information.     May be responsible for discharges, assessments, and clinical documentation as needed. 

CNA/Caregiver Part-Time

Take and record vital signs. Measure and record height, weight, and fluid intake/output. Recognize abnormal changes in body functioning and importance of reporting such changes to a supervisor. Assist as needed with medication reminders, bathing, grooming, dressing, escort service, and other activities of daily living. Transfer, position, and turn residents. Provide skin care. Contribute to the resident’s assessment and the plan of care. Provide nursing rehabilitation/restorative nursing services. Care for residents with dementia. Follow the schedule of resident’s needs set out by supervisor. Provide emotional and social support to residents. Promote and protect resident rights, assist residents to make informed decisions, treat residents with dignity and respect, protect resident’s personal belongings, report suspected abuse or neglect, avoid the need for physical restraints in accordance with the current professional standards, and support independent expression, choice, and decision-making consistent with applicable law and regulation. Inform supervisor of any resident issues or concerns. Respect and encourage the independence and dignity of the residents. Respect residents’ confidentiality. Familiar with emergency equipment and procedures. Understand duties during a fire drill and is familiar with evacuation plan.     Attend all required training, in-service, and staff meetings. Strive to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices. Maintain a positive and professional demeanor toward residents, visitors, families, and co-workers. Adhere to all policies and procedures of Senior Lifestyle Corporation. Perform other duties as assigned.
 

Assisted Living Community Relations Counselor

Meet all expectations of meaningful contacts, leases and occupancy. Respond and follow-up with all walk-ins, phone-ins, mail-ins timely and appropriately. Secure the required number of budgeted move ins and achieve net gains each month. Develop and maintain a good working relationship with residents, families, and professional providers of care.    Participate in the community outreach program to generate the necessary number of referrals to reach sales goals. Report all sales activity to supervisor daily. Assure every alternative is considered before closing a lead.  Treat each inquiry with value. Maintain confidentiality of all pertinent information.

Business Office Manager

Maintain basic knowledge of computer software and internet applications. Maintain resident, business office and employee files. Perform Human Resources function by conducting new hire orientation and administering benefits. Assist in answering resident billing issues. Also maintain the ancillary charge worksheet on a weekly basis. Ensure that move-in deposits and rents are deposited in a timely manner. Send monthly delinquent notices and follows collection guidelines. Process resident lease renewal letters. Ensure that payroll is accurately prepared and reported to the corporate office at the time designated. Ensure that accounts payable procedures are processed timely & accurately. Ensure that all proprietary, financial, and resident information is kept confidential. Monitor and record petty cash disbursements and reconcile to replenish.  Process/file workers compensation claims. Respect and maintain confidentiality of the office, it’s records, and restricted information. Understands the safety and disaster plan. Participate in the Manager on Duty program. Attend various community events. Attend all required training, in-service, and staff meetings. Executive Chef

Prepare meals from standardized recipes in accordance with order in a timely and attractive manner. Set up food service line and workstations with needed condiments. Ensure all condiments and garnishes that are required are prepared and served with the appropriate foods. Assist in recipe and menu development.  Coordinate prep for the following day’s menu. Lead a pre-meal meeting with food servers to review detail of daily menu.   Assist with training of new kitchen employees. Ensure an adequate number of culinary employees each shift and ensures absences are covered as to not allow coverage holes. Assist with scheduling of employees within the department.  Assist with food ordering, monitoring inventory and creating the menu cycle. Keep stock rooms, coolers, and freezers clean and ensure that food supply stocks are rotated and that all perishables are labeled, dated, and stored properly. Maintain kitchen cleanliness and food preparation according to state and local health department code requirements. Keep food waste to a minimum by utilizing food storage and food recycling techniques. Manage and supervise Sous Chefs, Cooks, Lead Cooks, Prep Cooks, and Utility Workers as needed.

Server

Accept food order, communicate to kitchen and deliver food and beverage to residents and/or guests in a timely manner. Ensure resident/guest satisfaction by responding quickly to needs and requests.  Actively buss and reset tables as well as complete assigned side work. Maintain a positive and professional demeanor toward all residents, visitors and co-workers.

Director of Dining Services

Follow Company Dining Policy and Procedures and while ensuring that all department personnel are compliant as well.  Responsible for the purchasing of all dining food and nonfood items and ensure all purchases for the department achieve 95% DSSI ordering compliance.  Balance and maintain budgets for raw food and supplies. Utilize company standard recipes when completing food production for meals. Ensure that food is served at proper temperature and in proper portion sizes to ensure food safety and palatability according to our food temperature policy.   Ensure that residents’ Prescribed Diets are offered when applicable. Drive high satisfaction by offering our residents choice, dignity and independence in dining according to our hospitality and customer service policy.  Ensure operating and cleaning instructions/procedures are developed and implemented for the department.  Perform Pre-meal Dining Stand-Up with service staff to review daily menu details. Know and adhere to all company and the local, state, and county health departments regulations and acts as the community’s dietary representative for any health department or other required inspections; assure compliance, follow-up to assure any deficiencies are immediately corrected. Ensure all dining areas are clean and safely operating, completes monthly Self Sanitation audit according to the sanitation audit tool and operation and sanitation policies.   Interview, manage, train, hire, discipline, and terminate all dining staff personnel as needed. Oversee the scheduling of all food service personnel to maintain compliance with labor budget.
