Director of Memory Care

Assess/evaluate potential residents for Memory Care level of care. This may involve assessing/evaluating potential residents in their home or another healthcare setting; must be able to drive to location and perform assessment/evaluation professionally and timely if necessary. Monitor documentation systems to ensure that all regulatory timeframes for review of assessments and service plans are met and that resident needs are met. Write and review residents’ assessments/evaluations and care-plans with appropriate staff. Coordinate, along with the Executive Director and Director of Health and Wellness, family/resident care conferences and serve as a direct liaison between residents, families, and staff. Work with residents, families, and other staff to ensure that residents are at appropriate levels of care. Conduct interviews, orientation, and ongoing training of Memory Care Team employees in accordance with employee training policy and regulatory guidelines.  Coach and counsel departmental employees in accordance with Senior Lifestyle human resources policies.  Manage Memory Care employees in partnership with the Health and Wellness director or designee.  Promote the company’s Embrace philosophy in all aspects of his/her role to include interactions with resident, family, and staff and the daily oversight of Memory Care operations in partnership with the Health and Wellness Director. Organize the monthly programming calendar and needed supplies.  Plan, coordinate, and evaluate Memory Care resident programs. Lead high quality innovative programs in Memory Care to include a minimum of 3-5 self-lead programs daily. Implement and monitor programs and processes to promote excellence in programming while maintaining a high level of resident/family/staff satisfaction in a fiscally responsible manner. Adhere to the budget guidelines for the department. Maintain current knowledge of Alzheimer’s/dementia topics and participates Memory Care Department Director training as assigned. Provide education, training, and hands on support to community staff.

Certified Nursing Assistant (CNA) – (EVENING WEEKEND)

$18-$22/HR Take and record vital signs. Measure and record height, weight, and fluid intake/output. Recognize abnormal changes in body functioning and importance of reporting such changes to a supervisor. Assist as needed with medication reminders, bathing, grooming, dressing, escort service, and other activities of daily living. Transfer, position, and turn residents. Provide skin care to residents. Contribute to the resident’s assessment and the plan of care. Provide nursing rehabilitation/restorative nursing services. Follow the schedule of resident’s needs set out by supervisor. Provide emotional and social support to residents. Promote and protect resident rights, assist residents in making informed decisions, treat residents with dignity and respect, protect resident’s personal belongings, report suspected abuse or neglect, avoid the need for physical restraints in accordance with the current professional standards, and support independent expression, choice, and decision-making consistent with applicable law and regulation. Inform supervisor of any resident issues or concerns. Respect and encourage the independence and dignity of the residents. Respect residents’ confidentiality.

Resident Program Coordinator

Plan, develop, and implement resident lifestyle programming within the community as well as through outside trips and events. Survey and interview residents to determine their past and present interests and talents, as well as their satisfaction with the community’s programs and activities. Recognize the individuality, diversity, and spirituality of all residents when planning activities and celebrating holiday functions. Develop relationships with residents to encourage their participation in activities that enhance their quality of and appreciation for life. Observe residents’ function and behavior and reports significant changes or concerns to appropriate staff. Utilize outside community resources and independent contractors to enrich the variety of programs made available to residents. Assist with developing and managing the annual department budget.   Assist with managing Resident Program Assistants as needed in the absence of the Director of Resident Programs.

Director of Health and Wellness

Directly oversee the delivery of care to Assisted Living residents to ensure that emotional, physical, psychological and safety needs are met through all program services and activities. Coordinate, along with the Executive Director, family/resident care conferences and serve as a direct liaison between residents, families, and staff.  Work with residents, families, and other staff to ensure that residents are at appropriate levels of care.  Manage labor costs, supplies, and other line items in clinical budget.  Monitor documentation systems to ensure that all regulatory timeframes for review of assessments and service plans are met and that resident needs are met. Write and review residents’ assessments/evaluations and care-plans with appropriate staff. Assess/evaluate potential residents for Assisted Living level of care.  This may involve assessing/evaluating potential residents in their home or another healthcare setting; must be able to drive to location and perform assessment/evaluation professionally and timely if necessary. Schedule and conduct new resident orientation for all Assisted Living Residents.  Provide, plan, schedule, and monitor staff training programs and in-services for the Assisted Living staff including statutory or regulatory required training. Attend all required training, in-service, and staff meetings. Manage and coordinate the work of all nursing and other assisted living staff, communicating pertinent resident health and safety information.  This includes overseeing and/or completing staff schedules monthly and daily assignments.  Strive to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices.  Ensure compliance with all state regulatory requirements including mandatory reporting to State. Ensure that all concern and incident reports are completed, filed and that follow-up is conducted appropriately.  Assist and/or perform in mock surveys.  Maintain a positive and professional demeanor toward residents, visitors, families, and co-workers while adhering to all policies, standards and procedures of Company. Require 24/7 on-call clinical support. Perform other duties as assigned.

Care Associate Wellness

Assisted Living Attendant/Caregiver

Provide assistance as needed with medication reminders (as applicable), bathing, grooming, dressing, escort service, and other activities of daily living. Provide emotional and social support to residents. Document daily log of assistance. Respect and encourage the independence and dignity of the residents. Respect residents’ confidentiality. Ensure that resident/family/employee concerns receive timely response and follow up, thus striving to provide a high level of customer satisfaction. Maintain a positive and professional demeanor toward all residents, visitors and co-workers.

Driver

Transport residents and perform community errands as assigned.  Ensure that all residents who use the transportation are returned safely to the building. Assist all residents in and out of the vehicle as needed.  Maintain cleanliness of vehicle by performing and/or scheduling regular interior/exterior cleaning. Report mechanical needs and repairs and assist in coordination to have repairs completed.

Executive Director- (Personal Care Administrator)

Act as the on-site executive for all operations, including being the main point of contact for all staff, residents, prospects, community organizations, government agencies and the public when needed.  Maintains resident retention by working with residents and their families, dealing with any issues that arise. Responds and follows up with all walk-ins, phone-ins, mail-ins timely and appropriately. Keeps up to date with information about competitors. Meets all expectations of meaningful contacts, leases and occupancy. Maintain budget accountability and cash flow; aggressively anticipate and minimize negative budget variances and deficits. Hire, train, discipline, and terminate employees in accordance with Senior Lifestyle Corporate policy.  Maintain all local, state, and federal licenses for the community. Lead staff meetings.  Promote and protect resident rights; assisting residents to make informed decisions and treating them with dignity and respect.  Become an intricate part of the community in social and civic affairs by representing the community in local, state, and professional organizations.  Manage other support level and management roles as needed. 

Certified Medication Aide – Assisted Living (AL)

Responsible for supervision of self-administered medication for the Assisted Living Department in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines as directed by the LPN/LVN/RN and/or the Assisted Living Director/Health and Wellness Director. Count and sign on approved form, all narcotics with approved personnel before accepting medication keys.  Contact a supervisor immediately with discrepancies. Order all necessary medication and treatments from appropriate pharmacy or notify responsible party when supply is no less than 7 days. Notify supervisor when supply is less than 2 days and medicine hasn’t been received. Responsible for receiving and verifying all medicine received is accurate according to pharmacy label and MAR.  Notifies pharmacy and DH&W of any discrepancies. Responsible for proper storage of all medications and ensuring all lotions, ointments and liquids stored separately from oral medications. Responsible for dating all appropriate vials or bottles at time of initial opening and following pharmacy protocol on length of time they can be used. Responsible for weekly inspections of all medications to ensure all expired medications are removed. Responsible for verifying all current MARs are compared to new MARs at change over time and notifying the supervisor of any discrepancies. Responsible for removing all discontinued medication from med cart and giving it to the supervisor. Provide personal care to residents to include, but not limited to, bathing, skin care, oral hygiene, grooming, dressing, incontinence care, and eating, etc. Provide general nursing care to include, but not limited to, vital signs, temperatures, blood pressure, and weights. Document appropriate information in the daily log as required and notify supervisor of all concerns. Answer calls and assist residents with needs as assigned. Responsible for daily cleaning of all medicine carts at end of shift and restocking as needed.  Responsible for weekly deep cleaning of all med carts. Attend and participate in staff training and in-services. Must maintain accurate records to include preparation of all medical charts for new admissions and readmissions and ensures medical record is complete within forty-eight (48) hours as Policy and Procedure (P&P) and regulatory guidelines.
