Director of Health and Wellness

Directly oversee the delivery of care to Assisted Living residents to ensure that emotional, physical, psychological and safety needs are met through all program services and activities. Coordinate, along with the Executive Director, family/resident care conferences and serve as a direct liaison between residents, families, and staff.  Work with residents, families, and other staff to ensure that residents are at appropriate levels of care.  Manage labor costs, supplies, and other line items in clinical budget.  Monitor documentation systems to ensure that all regulatory timeframes for review of assessments and service plans are met and that resident needs are met. Write and review residents’ assessments/evaluations and care-plans with appropriate staff. Assess/evaluate potential residents for Assisted Living level of care.  This may involve assessing/evaluating potential residents in their home or another healthcare setting; must be able to drive to location and perform assessment/evaluation professionally and timely if necessary. Schedule and conduct new resident orientation for all Assisted Living Residents.  Provide, plan, schedule, and monitor staff training programs and in-services for the Assisted Living staff including statutory or regulatory required training. Attend all required training, in-service, and staff meetings. Manage and coordinate the work of all nursing and other assisted living staff, communicating pertinent resident health and safety information.  This includes overseeing and/or completing staff schedules monthly and daily assignments.  Strive to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices.  Ensure compliance with all state regulatory requirements including mandatory reporting to State. Ensure that all concern and incident reports are completed, filed and that follow-up is conducted appropriately.  Assist and/or perform in mock surveys.  Maintain a positive and professional demeanor toward residents, visitors, families, and co-workers while adhering to all policies, standards and procedures of Company. Require 24/7 on-call clinical support. Perform other duties as assigned.

Private Restaurant Line Cook

Prepare all food using the daily planner and SLC standard recipes. Work the food line during mealtimes. Ensure that all food meets safety guidelines and that all kitchen areas meet sanitation guidelines. Ensure resident and guest satisfaction by responding quickly to needs and requests.  Maintain a positive and professional demeanor toward all residents, visitors and co-workers. $20.00 to $22.00 an hour

Executive Director

Act as the on-site executive for all operations, including being the main point of contact for all staff, residents, prospects, community organizations, government agencies and the public when needed.  Maintains resident retention by working with residents and their families, dealing with any issues that arise. Responds and follows up with all walk-ins, phone-ins, mail-ins timely and appropriately. Keeps up to date with information about competitors. Meets all expectations of meaningful contacts, leases and occupancy. Maintain budget accountability and cash flow; aggressively anticipate and minimize negative budget variances and deficits. Hire, train, discipline, and terminate employees in accordance with Senior Lifestyle Corporate policy.  Maintain all local, state, and federal licenses for the community. Lead staff meetings.  Promote and protect resident rights; assisting residents to make informed decisions and treating them with dignity and respect.  Become an intricate part of the community in social and civic affairs by representing the community in local, state, and professional organizations.  Manage other support level and management roles as needed. 

Janitor

Keep all public areas of the community clean and tidy.  Remove loose garbage from the ground and waste baskets. Assist in cleaning apartments when they transition from a former resident to a new resident. Use cleaning chemicals safely and properly as needed. 

Business Office Manager – SNF

Responsible for all daily and monthly accounts receivable, including but not limited to: Billing and collections of all Medicaid, Medicare, HMO, Veterans Administration, and private pay accounts Maintains all resident files, ensuring financial and resident information is kept confidential Maintains and records notes in collection log and in billing computer systems on all past due balances Ensures compliance with company collection policies by issuing delinquent notices and forwarding past-due accounts to the collection agency in accordance with established procedures Ensure timely and accurate resident financial records and trust fund management Data entry of resident information into billing computer software upon admission and discharge Maintains and records daily census – admissions and discharges, payer changes, and room changes – ensuring proper documentation is made into billing computer system Responsible for preparing daily bank deposits and daily cash posting of resident accounts into billing computer software Process resident rate increases letters as directed by corporate team Work closely with admissions, nursing, and corporate team to ensure smooth operations Perform insurance verifications to analyze eligibility and benefit/copay information related to current and discharged admissions Responsible for all accounts receivable outcomes by maintaining DSO that is within the goals of company Assists in answering general resident, family, and vendor questions in a timely manner Responsible for all month-end close procedures and deadlines as directed by corporate team Maintain compliance with all federal, state, and company financial policies Utilizes Point Click Care, Microsoft Excel, Microsoft Word, and other computer/systems to ensure accurate work is completed in an organized, timely, and professional manner Develops and maintains expert knowledge of Point Click Care computer System and keeps current with updates Participates in manager on duty program. Attends various community events Attends all required training, in-service, and staff meetings Adheres to all policies and procedures of Senior Lifestyle Corporation Respect and maintain confidentiality of the office, its records and restricted information Maintains a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices Performs other duties as assigned.

Property Manager

Act as main point of contact for staff, residents, prospects, community organizations, government agencies, and the public. Lead the property and make decisions as the property’s main stakeholder. Optimize occupancy at the property by working with prospective residents, existing residents, family members, and guests.   Hire, train, and manage all staff at the property. Maintain compliance of resident records with city, state, and federal programs. Manage the property’s budget and cash flow. Ensure building, grounds, and property are maintained at the highest standard of excellence through preventative maintenance systems and programs.

Cook/Part Time & Weekends-$17/hourly

Maintenance Assistant-$17/hr.

Salary range $17-$19 per hour Complete work orders around the community. Perform work and repairs needed to ready apartments prior to move-ins. Order and organize maintenance supplies as needed.  Complete general maintenance projects, including installations and repairs involving plumbing, electrical equipment, carpentry, painting, equipment repair, snow removal, and landscaping.

STNA 7PM – 7AM (Assisted Living)

Take and record vital signs. Measure and record height, weight, and fluid intake/output. Recognize abnormal changes in body functioning and importance of reporting such changes to a supervisor. Assist as needed with medication reminders, bathing, grooming, dressing, escort service, and other activities of daily living. Transfer, position, and turn residents. Provide skin care. Contribute to the resident’s assessment and the plan of care. Provide nursing rehabilitation/restorative nursing services. Care for residents with dementia. Follow the schedule of resident’s needs set out by supervisor. Provide emotional and social support to residents. Promote and protect resident rights, assist residents to make informed decisions, treat residents with dignity and respect, protect resident’s personal belongings, report suspected abuse or neglect, avoid the need for physical restraints in accordance with the current professional standards, and support independent expression, choice, and decision-making consistent with applicable law and regulation. Inform supervisor of any resident issues or concerns. Respect and encourage the independence and dignity of the residents. Respect residents’ confidentiality. Familiar with emergency equipment and procedures. Understand duties during a fire drill and is familiar with evacuation plan.     Attend all required training, in-service, and staff meetings.
