Dietary Aide

Follow and perform duties in adherence to Senior Lifestyle Corporation Dining Policy and Procedures according to the Dining Services Policy and Procedure Manual.   Serve food in accordance with established portion control procedures and ensures that foods follow prescribed modified or therapeutic diets, nutrition/hydration requirements and resident preferences.   Assist or serve meals as necessary and in a timely manner assuring that they are palatable and appetizing in appearance as well as accurate in content to the resident.  This will include resident room service as well as service to designated dining areas.  Setup, deliver, and verify accuracy of meal trays, food carts, and dining areas as instructed.  Upon meal completion team member will remove all soiled items/equipment and deliver to the dishwashing area.  Distribute and collect menus as necessary for each meal period. Assist department in obtaining proper food supplies for the next meal.  Assist cooks in preparation of menu items. Prepare and deliver snacks and other items as instructed and assist in inventory control and stocking. All Dietary Department personnel should follow safety precautions to prevent accidents in food preparation according to our accident prevention in food preparation and prevention of back injury policy. Perform kitchen utility duties such as, but not limited to: sweep and mop floors, buss items from dining areas to dish room, wash and clean small wares and equipment. Clean and sanitize work areas, cutting boards, and table tops as well as removing garbage. Keep work areas, clean, dry and free of hazards. Know and adhere to all company and the local, state, and county health department’s regulations. Ensure that all dining areas are clean and safely operating.   Attend all required training, in services, and staff meetings.  Meet all department training standards through Redi-Learning and/or other necessary training modules as required.

Registered Nurse – Skilled Nursing – Evening Shift

Responsible for medication supervision/administration in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines. May be responsible for the admissions of new residents to include all admission assessments and documentation. Responsible for completing acute and long-term care plans or service plans.  May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the Director of Nursing. May be responsible for discharges, assessments, and clinical documentation as indicated.  Provide general consultation regarding health-related concerns within the community. Notify physicians and/or family members of any change in resident’s health and provides proper documentation. Respond quickly to all emergencies. Maintain close communication with all departments and staff members. Maintain confidentiality of residents’ and prospective residents’ information. Understand role in the Safety & Disaster Plan. Provide educational meetings for residents and staff.

PSS / Care Associate – Memory Care (MC) – Evening Shift

Provide kind, caring, and consistent companionship and support to residents. Provide assistance, encouragement, and restorative interventions, as needed, for Memory Care residents to be as independent as possible in their activities of daily living including but not limited to: bathing, dressing, grooming, toileting, feeding, and mobility. Participate in and lead one on one and group activities. Serve residents during mealtime, assist with memory care laundry, and basic housekeeping within the memory care neighborhood. Ensure that resident/family/employee concerns receive timely response and follow up, thus striving to provide a high level of customer satisfaction. Maintain a positive and professional demeanor toward all residents, visitors and co-workers.

PSS / Care Associate – Memory Care (MC) – Day Shift

PSS – Assisted Living (AL) – Full Time / Part Time – All Shifts

Provide assistance as needed with medication reminders, bathing, grooming, dressing, escort service, and other activities of daily living. Follow the schedule of resident’s needs set out by supervisor. Provide emotional and social support to residents. Document daily log of assistance. Inform supervisor of any resident issues or concerns. Respect and encourage the independence and dignity of the residents. Respect residents’ confidentiality.

Manager Nurse – Assisted Living

Provide direct and individualized nursing care to assigned residents based on established nursing protocol. Observe and assess for: pain/discomfort, oxygen therapy, all drainage, all dressings, and any incontinent elimination which may have occurred.  Provide ignition and ongoing systematic assessment of the physical, emotional, social, educational, and functional needs of the resident/family through observation, interview, and physical examination.  Interpret assessment data and information to assist in developing a plan of care.  Assure medical orders are transcribed and processed accurately and timely and integrate the medical care plan into the provision of nursing care. Assure that medication administration is correct, according to the established nursing policies and procedures. Provide accurate and timely documentation consistent with the resident plan of care.  Administer medication per physician orders. Document properly on MOR. Observe for any adverse reaction.  Assemble resident charts, resident records, and assure all necessary form are completed in a timely manner. Assure all monthly summaries are completed in a timely manner for assigned residents.  Prepare incident/accident reports for residents as needed per policy, notify AL Director, physician, and family post incident per policy and document accordingly.  Assist resident in learning self-care activities.  Supervise all resident assignments to nursing team members, and assure special instructions are given to team members.  Observe and supervise all meals to ensure residents receive the proper diet and any assistance required.  Monitor and organize all material resources necessary for meeting needs of assigned unit.  Recognize emergencies and life-threatening situations and initiate appropriate action. Ensure that resident call lights are promptly answered, and their needs are addressed timely and appropriately.  Assist with the orientation of new staff members.  Ensure that all scheduled activities take place on assigned unit by utilizing activity personnel and/or nursing personnel. 

MDS Coordinator / Nurse Assessment Coordinator

Coordinate the assessment schedule and opens resident assessments in accordance with current Federal and state rules, regulations and guidelines that govern the resident assessment instrument, including the implementation of CAAs and CATS. Select the Assessment Reference Date (ARD) that will most accurately reflect the resident’s condition based upon the capture of all resources utilized and care provided.  Gather the necessary documentation to accurately encode the assessment item set and ensure that it is complete, accurate, signed and submitted to the state database in accordance with RAI or state regulations. Meet daily with the Rehab Program Manager to review Resident therapy schedules in order to minimize the need for non-scheduled assessments. Coordinate with the interdisciplinary team to ensure the care plan is comprehensive, completed timely, and remains current based on the Resident’s condition.  Attend and participate in the Resident Care Conference. Disseminate any new or updated materials involving the RAI process to the interdisciplinary team to include assessment calendars, due dates, etc. Facilitate the involvement of appropriate health professionals as needed to improve or maintain the Resident’s functional abilities at the highest practicable level.  Assist and/or direct Managed Care case management duties. Review the state validation reports and ensure that the appropriate follow-up action is taken, if needed. Serve on, participate in, and attend various other committees of the facility (e.g. At Risk, Triple Check, Medicare UR) as required, and as directed by the Administrator or Executive Director. Communicate with and provide education to all nurses and CNAs to expand their knowledge of the RAI process and how their duties can impact the process.   Communicate with Business Office regarding unfinished assessments, defaults, and other billing concerns. Understand, comply with, and promote all rules and regulations regarding Residents’ Rights; promote positive relationships with Residents, visitors, and regulators, to include presenting a professional appearance.  Maintain HIPAA compliance at all times.

Social Services Assistant

Assist with assessments and identify resident’s psychosocial needs and facilitate methods of meeting with those needs.  Assist with promoting staff sensitivity to the unique needs of the residents by education staff regarding possible reasons for resident’s feelings and behaviors when needed.  Assist with assessments and complete the sections of the Minimum Data Sheet (MDS) assigned completely, timely, and accurately.  Assist with documents of the social care component of the Comprehensive Care Plan for each resident as identified by the interdisciplinary team with seven days of completion of an MDS.  Assist with understanding and actively promote use of the problem-solving process to obtain positive resident outcome.  Assist with the understanding of the residents’ rights and utilize this in practice of service delivery.  Assist with and act as a liaison with social, health, and community agencies.  Assist with the uses of appropriate counseling techniques to affect a positive change in resident’s psychosocial status.  Assist with developing and maintaining contacts with hospitals to increase admission applicants.  Attend care plan meetings to provide professional guidance to improve the quality of services rendered to residents and families.  Assist with the admission process as needed.  Assist with marketing the facility to outside contacts.  Assist with contacting the families regarding resident needs. 

Certified Nursing Assistant (CNA) – Skilled Nursing

Take and record vital signs. Measure and record height, weight, and fluid intake/output. Recognize abnormal changes in body functioning and importance of reporting such changes to a supervisor. Assist as needed with medication reminders, bathing, grooming, dressing, escort service, and other activities of daily living. Transfer, position, and turn residents. Provide skin care to residents. Contribute to the resident’s assessment and the plan of care. Provide nursing rehabilitation/restorative nursing services. Follow the schedule of resident’s needs set out by supervisor. Provide emotional and social support to residents. Promote and protect resident rights, assist residents in making informed decisions, treat residents with dignity and respect, protect resident’s personal belongings, report suspected abuse or neglect, avoid the need for physical restraints in accordance with the current professional standards, and support independent expression, choice, and decision-making consistent with applicable law and regulation. Inform supervisor of any resident issues or concerns. Respect and encourage the independence and dignity of the residents. Respect residents’ confidentiality.
