PT Memory Care Coordinator

Promote the company’s Embrace philosophy in all aspects of his/her role for residents, families and staff. Implement and oversee the six unique pillars of Embrace. Daily oversight of the operations in the Memory Care area in partnership with the Memory Care Director and Health and Wellness team.  Lead high-quality innovative programs in Memory Care. This is the positions primary focus. Person will personally lead a minimum of 5-7 programs per day. Assist in planning, coordinating, and evaluating resident activities. Encourage resident participation in programs. Provide education, training, and hands on support to community staff in partnership with the community managers. Implement and monitor programs and process to promote excellence in programming while maintaining a high level of resident/family/staff satisfaction in a fiscally responsible manner.

PRN QMAP – Memory Care

We are seeking a compassionate and detail-oriented Full Time QMAP (Qualified Medication Administration Personnel) to join our Memory Care team in Colorado Springs, United States. In this role, you will play a crucial part in providing high-quality care and support to residents with memory-related conditions such as Alzheimer’s and dementia. Administer medications to residents according to prescribed orders and facility protocols Monitor residents for medication side effects and report any concerns to the nursing staff Maintain accurate and up-to-date medication administration records Assist with daily living activities, including personal care and grooming Engage residents in memory-enhancing activities and social interactions Collaborate with interdisciplinary team members to ensure comprehensive resident care Respond promptly to resident needs and emergency situations Maintain a clean, safe, and comfortable environment for residents Adhere to all state regulations and facility policies regarding medication administration and resident care

PRN Caregiver – Memory Care

Provide kind, caring, and consistent companionship and support to residents. Provide assistance, encouragement, and restorative interventions, as needed, for Memory Care residents to be as independent as possible in their activities of daily living including but not limited to: bathing, dressing, grooming, toileting, feeding, and mobility. Participate in and lead one on one and group activities. Serve residents during mealtime, assist with memory care laundry, and basic housekeeping within the memory care neighborhood. Ensure that resident/family/employee concerns receive timely response and follow up, thus striving to provide a high level of customer satisfaction. Maintain a positive and professional demeanor toward all residents, visitors and co-workers.

FT Dishwasher

Maintain sanitary condition of dishes, utensils, glasses, pots, pans, equipment and kitchen in accordance with Senior Lifestyle Corporation standards. Safely and efficiently operate dish washing machine according to procedure manual. Ensure proper soap and rinse chemicals are loaded for washing. Remove from service any dishes, utensils, and glassware that are damaged or broken. Stock delivered food and non-food items. Empty and clear garbage bins and breakdown of boxes. Deep cleaning of kitchen and equipment.     Attend all training and in-service seminars. Maintain a clean and sanitary work environment. Perform all duties in adherence to Senior Lifestyle Corporation standards. Strive to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices. Maintain a positive and professional demeanor toward all residents, visitors and co-workers.

Sous Chef

Prepare meals from standardized recipes in accordance with order in a timely and attractive manner. Set up food service line and workstations with needed condiments. Ensure all condiments and garnishes that are required are prepared and served with the appropriate foods. Assist in coordinating prep for the following day’s menu. Participate in a pre-meal meeting with food servers to review detail of daily menu.   Assist with training of new kitchen employees. Assist with scheduling of employees within the department.  Assist with food ordering, monitoring inventory and creating the menu cycle. Keep stock rooms, coolers, and freezers clean and ensure that food supply stocks are rotated and that all perishables are labeled, dated, and stored properly. Maintain kitchen cleanliness and food preparation according to state and local health department code requirements. Keep food waste to a minimum by utilizing food storage and food recycling techniques. Manage and supervise Cooks, Lead Cooks, Prep Cooks, and Utility Workers as needed.

Dietary Aide – Skilled Nursing Facility

Follow and perform duties in adherence to Senior Lifestyle Corporation Dining Policy and Procedures according to the Dining Services Policy and Procedure Manual.   Serve food in accordance with established portion control procedures and ensures that foods follow prescribed modified or therapeutic diets, nutrition/hydration requirements and resident preferences.   Assist or serve meals as necessary and in a timely manner assuring that they are palatable and appetizing in appearance as well as accurate in content to the resident.  This will include resident room service as well as service to designated dining areas.  Setup, deliver, and verify accuracy of meal trays, food carts, and dining areas as instructed.  Upon meal completion team member will remove all soiled items/equipment and deliver to the dishwashing area.  Distribute and collect menus as necessary for each meal period. Assist department in obtaining proper food supplies for the next meal.  Assist cooks in preparation of menu items. Prepare and deliver snacks and other items as instructed and assist in inventory control and stocking. All Dietary Department personnel should follow safety precautions to prevent accidents in food preparation according to our accident prevention in food preparation and prevention of back injury policy. Perform kitchen utility duties such as, but not limited to: sweep and mop floors, buss items from dining areas to dish room, wash and clean small wares and equipment. Clean and sanitize work areas, cutting boards, and table tops as well as removing garbage. Keep work areas, clean, dry and free of hazards. Know and adhere to all company and the local, state, and county health department’s regulations. Ensure that all dining areas are clean and safely operating.   Attend all required training, in services, and staff meetings.  Meet all department training standards through Redi-Learning and/or other necessary training modules as required.

FT Server

Accept food order, communicate to kitchen and deliver food and beverage to residents and/or guests in a timely manner. Ensure resident/guest satisfaction by responding quickly to needs and requests.  Actively buss and reset tables as well as complete assigned side work. Maintain a positive and professional demeanor toward all residents, visitors and co-workers.

FT Living Well Attendant

Provide assistance as needed with medication reminders, bathing reminders, assistance with grooming and dressing, companionship, and other services to promote living well and independence. Follow the schedule of resident’s needs set out by supervisor. Provide emotional and social support to residents. Document daily log of assistance. Respect and encourage the independence and dignity of the residents. Respect residents’ confidentiality. Ensure that resident/family/employee concerns receive timely response and follow up, thus striving to provide a high level of customer satisfaction. Maintain a positive and professional demeanor toward all residents, visitors and co-workers.

PRN Registered Nurse (RN)

Responsible for medication supervision/administration for the Skilled Nursing Community in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines. Provide general consultation regarding health-related concerns within the community. Notify physicians and/or family members of any change in resident’s health and provides proper documentation. Respond quickly to all emergencies. Maintain close communication with all departments and staff members. Maintain confidentiality of residents’ and prospective residents’ information. Provide educational meetings for residents and staff.

Full-time Dayshift QMAP – Assisted Living

Responsible for the safe administration of medications ensuring the 6 rights of medication administration are followed. Count and document all narcotics that are administered to residents.  Responsible for the total inventory of medications at the community; including but not limited to ordering medications, receiving and verifying medications, storing and dating medications, removing expired or discontinued medications, and notifying the proper parties when medication supply is low. Notice discrepancies regarding medication inventory and report to the appropriate parties.  Responsible for daily cleaning of all medicine carts at end of shift and restocking as needed. Responsible for weekly deep cleaning of all med carts. Responsible for verifying all current MARs are compared to new MARs at change over time. Provide personal care to residents to residents as needed, including bathing, skin care, oral hygiene, grooming, dressing, incontinence care, and eating. Provide general nursing care to include, but not limited to, vital signs, temperatures, blood pressure, and weights. Document appropriate information in the daily log as required.
