Director of Memory Care

Conduct interviews, orientation, and ongoing training of Memory Care Team employees in accordance with employee training policy and regulatory guidelines.  Coach and counsel departmental employees in accordance with Senior Lifestyle human resources policies.  Manage Memory Care employees in partnership with the Health and Wellness director or designee.  Promote the company’s Embrace philosophy in all aspects of his/her role to include interactions with resident, family, and staff and the daily oversight of Memory Care operations in partnership with the Health and Wellness Director. Organize the monthly programming calendar and needed supplies.  Plan, coordinate, and evaluate Memory Care resident programs. Lead high quality innovative programs in Memory Care to include a minimum of 3-5 self-lead programs daily. Implement and monitor programs and processes to promote excellence in programming while maintaining a high level of resident/family/staff satisfaction in a fiscally responsible manner. Adhere to the budget guidelines for the department. Maintain current knowledge of Alzheimer’s/dementia topics and participates Memory Care Department Director training as assigned. Provide education, training, and hands on support to community staff.
 

Staffing Manager

Maintain staffing operations within health and wellness department. Develop work schedules by allocating employees in shifts and positions. Secure coverage for vacancy shifts. Monitor overtime accumulation and attendance of employees. Support Department Managers in candidate screening and hiring processes. Provide community tours to newly hired employees. Assist Human Resources in preparation and execution of orientation and training plans. Assume responsibility for timekeeping and time off requests. Work with the HR Coordinator to verify payroll accuracy.  Ensure compliance with internal and external policies and regulations; to include ongoing education, and compliance with certifications/license, CPR, First Aide, TB, Fingerprint Clearance Card. Prepare and submit reports on staffing operations. Manage staffing software utilizing ADP. Must be available for after-hour calls from staff regarding schedules. Obtain Certified Medical Technician certification and pass Direct Care Test for availability to schedule yourself and fill in on the floor. Attend various community events.

Server – FT/PT

Accept food order, communicate to kitchen and deliver food and beverage to residents and/or guests in a timely manner. Ensure resident/guest satisfaction by responding quickly to needs and requests.  Actively buss and reset tables as well as complete assigned side work. Maintain a positive and professional demeanor toward all residents, visitors and co-workers.

Certified Medication Technician

Responsible for the safe administration of medications ensuring the 6 rights of medication administration are followed. Count and document all narcotics that are administered to residents.  Responsible for the total inventory of medications at the community; including but not limited to ordering medications, receiving and verifying medications, storing and dating medications, removing expired or discontinued medications, and notifying the proper parties when medication supply is low. Notice discrepancies regarding medication inventory and report to the appropriate parties.  Responsible for daily cleaning of all medicine carts at end of shift and restocking as needed. Responsible for weekly deep cleaning of all med carts. Responsible for verifying all current MARs are compared to new MARs at change over time. Provide personal care to residents to residents as needed, including bathing, skin care, oral hygiene, grooming, dressing, incontinence care, and eating. Provide general nursing care to include, but not limited to, vital signs, temperatures, blood pressure, and weights. Document appropriate information in the daily log as required.

Utility Worker/Dishwasher

Maintain sanitary condition of dishes, utensils, glasses, pots, pans, equipment and kitchen in accordance with Senior Lifestyle Corporation standards. Safely and efficiently operate dish washing machine according to procedure manual. Ensure proper soap and rinse chemicals are loaded for washing. Remove from service any dishes, utensils, and glassware that are damaged or broken. Stock delivered food and non-food items. Empty and clear garbage bins and breakdown of boxes. Deep cleaning of kitchen and equipment.     Attend all training and in-service seminars. Maintain a clean and sanitary work environment. Perform all duties in adherence to Senior Lifestyle Corporation standards. Strive to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices. Maintain a positive and professional demeanor toward all residents, visitors and co-workers.

Driver/Maintenance Assistant

Transport residents and perform community errands as assigned.  Ensure that all residents who use the transportation are returned safely to the building. Assist all residents in and out of the vehicle as needed.  Maintain cleanliness of vehicle by performing and/or scheduling regular interior/exterior cleaning. Report mechanical needs and repairs and assist in coordination to have repairs completed. Additional Maintenance duties as needed.

Resident Program Assistant Memory Care

Assist in planning and coordinating all resident activities.  Encourage residents to participate in programs and activities and provide transportation to them as needed. Lead various activities as directed by the Resident Program Director and Memory Care Director. Order and maintain supplies for activities as needed.

Licensed Practical Nurse Supervisor

Responsible for medication administration for the residents in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines.  May be responsible for the admissions of new residents to include all admission assessments and documentation.  Responsible for completing acute and long-term care plans or service plans.  May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the director.  Provide general consultation regarding health-related concerns within the community.  Notify physicians and/or family members of any change in resident’s health and provides proper documentation.  Respond quickly to all emergencies.  Maintain close communication with all departments and staff members.  Provide educational meetings for residents and staff.  Maintain confidentiality of residents’ and prospective residents’ information.  Attend all required training, in-service, and staff meetings.  Strive to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices. 

Personal Care Director – R.N./LPN

• Welcomes and becomes familiar with all residents and families on assisted services and 
acts as their liaison and advocate.
• Assists incoming residents, transfers and families with lease signing for assisted living.
• Directly oversees the delivery of care to Assisted Living residents to ensure that 
emotional, physical, psychological, and safety needs are met through all program 
services and activities.
• Coordinates family/resident care conferences and serves as a direct liaison between 
residents, families, and staff.
• Addresses and follows up on all assisted living resident concerns and satisfactions 
surveys.
• Works with residents, families, and other staff to ensure that residents are at appropriate 
levels of care.
• Develops and/or monitors documentation systems to ensure that all regulatory time
frames for review of assessments and service plans are met.
• Schedules and conducts new resident orientation for all Assisted Living Residents.
• Participates in interviewing, hiring, orientation, disciplining and terminating Assisted 
Living staff.
• Plans and schedules training programs and in-services for the Assisted Living staff.
• Ensures compliance with all state regulatory requirements including mandatory 
reporting.
• Ensures that all concern and incident reports are completed and filed, and that
appropriate follow-up is conducted.
• Manages and coordinates the work of all nursing and other assisted living staff, 
communicating pertinent resident health and safety information.
• Manages labor costs, supplies and other line items in Assisted Living budget.
• Participates in Manager on Duty rotation as assigned.
• Performs other duties as assigned.
