PT QMAP – Assistant Living

Responsible for the safe administration of medications ensuring the 6 rights of medication administration are followed. Count and document all narcotics that are administered to residents.  Responsible for the total inventory of medications at the community; including but not limited to ordering medications, receiving and verifying medications, storing and dating medications, removing expired or discontinued medications, and notifying the proper parties when medication supply is low. Notice discrepancies regarding medication inventory and report to the appropriate parties.  Responsible for daily cleaning of all medicine carts at end of shift and restocking as needed. Responsible for weekly deep cleaning of all med carts. Responsible for verifying all current MARs are compared to new MARs at change over time. Provide personal care to residents to residents as needed, including bathing, skin care, oral hygiene, grooming, dressing, incontinence care, and eating. Provide general nursing care to include, but not limited to, vital signs, temperatures, blood pressure, and weights. Document appropriate information in the daily log as required.

Cook – Part-Time – 11am – 730pm – Starting Wage $17.50

Prepare all food using the daily planner and SLC standard recipes. Work the food line during mealtimes. Ensure that all food meets safety guidelines and that all kitchen areas meet sanitation guidelines. Ensure resident and guest satisfaction by responding quickly to needs and requests.  Maintain a positive and professional demeanor toward all residents, visitors and co-workers.

Licensed Practical Nurse (LPN) – Memory Care

Compensation: Starting rate $35.00 Responsible for medication administration for the residents in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines. May be responsible for the admissions of new residents to include all admission assessments and documentation. Responsible for completing acute and long-term care plans or service plans. May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the director. Provide general consultation regarding health-related concerns within the community. Notify physicians and/or family members of any change in resident’s health and provides proper documentation. Respond quickly to all emergencies. Maintain close communication with all departments and staff members. Provide educational meetings for residents and staff. Understand roll in the Safety & Disaster Plan. Maintain confidentiality of residents’ and prospective residents’ information.     May be responsible for discharges, assessments, and clinical documentation as needed. 

Server

Accept food order, communicate to kitchen and deliver food and beverage to residents and/or guests in a timely manner. Ensure resident/guest satisfaction by responding quickly to needs and requests.  Actively buss and reset tables as well as complete assigned side work. Maintain a positive and professional demeanor toward all residents, visitors and co-workers. Pay Range: $15.50-$16.00/hr

PRN Resident Program Assistant

Assist in planning and coordinating all resident activities.  Encourage residents to participate in programs and activities and provide transportation to them as needed. Lead various activities as directed by the Resident Programs Director. Order and maintain supplies for activities as needed.

Cook

Housekeeper

General cleaning of resident apartments, hallways and other common areas. Assist in preparing empty apartments for resident move-ins. Ensure resident/guest satisfaction by responding quickly to needs and requests.  You may be asked to help respond to resident emergencies. Maintain a positive and professional demeanor toward all residents, visitors and co-workers. Pay Range is $17-$19/hr

Dietary Aide – Skilled Nursing Facility

Follow and perform duties in adherence to Senior Lifestyle Corporation Dining Policy and Procedures according to the Dining Services Policy and Procedure Manual.   Serve food in accordance with established portion control procedures and ensures that foods follow prescribed modified or therapeutic diets, nutrition/hydration requirements and resident preferences.   Assist or serve meals as necessary and in a timely manner assuring that they are palatable and appetizing in appearance as well as accurate in content to the resident.  This will include resident room service as well as service to designated dining areas.  Setup, deliver, and verify accuracy of meal trays, food carts, and dining areas as instructed.  Upon meal completion team member will remove all soiled items/equipment and deliver to the dishwashing area.  Distribute and collect menus as necessary for each meal period. Assist department in obtaining proper food supplies for the next meal.  Assist cooks in preparation of menu items. Prepare and deliver snacks and other items as instructed and assist in inventory control and stocking. All Dietary Department personnel should follow safety precautions to prevent accidents in food preparation according to our accident prevention in food preparation and prevention of back injury policy. Perform kitchen utility duties such as, but not limited to: sweep and mop floors, buss items from dining areas to dish room, wash and clean small wares and equipment. Clean and sanitize work areas, cutting boards, and table tops as well as removing garbage. Keep work areas, clean, dry and free of hazards. Know and adhere to all company and the local, state, and county health department’s regulations. Ensure that all dining areas are clean and safely operating.   Attend all required training, in services, and staff meetings.  Meet all department training standards through Redi-Learning and/or other necessary training modules as required.
