Cook (Full Time – Evening) (Assisted Living)

Prepare all food using the daily planner and SLC standard recipes. Work the food line during mealtimes. Ensure that all food meets safety guidelines and that all kitchen areas meet sanitation guidelines. Ensure resident and guest satisfaction by responding quickly to needs and requests.  Maintain a positive and professional demeanor toward all residents, visitors and co-workers.

HHA – Part Time

Provide assistance as needed with medication reminders, bathing, grooming, dressing, escort service, and other activities of daily living. Follow the schedule of resident’s needs set out by supervisor. Provide emotional and social support to residents. Document daily log of assistance. Inform supervisor of any resident issues or concerns. Respect and encourage the independence and dignity of the residents. Respect residents’ confidentiality.

Lead Cook

Prepare meals from standardized recipes in accordance with order in a timely and attractive manner. Set up food service line and workstations with needed condiments. Ensure all condiments and garnishes that are required are prepared and served with the appropriate foods. Assist in coordinating prep for the following day’s menu. Assist in training of new kitchen employees. Assist in food ordering and creating the menu cycle. Keep stock rooms, coolers, and freezers clean and ensure that food supply stocks are rotated and that all perishables are labeled, dated, and stored properly. Maintain kitchen cleanliness and food preparation according to state and local health department code requirements. Keep food waste to a minimum by utilizing food storage and food recycling techniques. Manage and supervise Cooks, Prep Cooks, and Utility Workers as needed.

Licensed Practical Nurse (LPN)

  Responsible for medication administration for the residents in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines. May be responsible for the admissions of new residents to include all admission assessments and documentation. Responsible for completing acute and long-term care plans or service plans. May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the director. Provide general consultation regarding health-related concerns within the community. Notify physicians and/or family members of any change in resident’s health and provides proper documentation. Respond quickly to all emergencies. Maintain close communication with all departments and staff members. Provide educational meetings for residents and staff. Maintain confidentiality of residents’ and prospective residents’ information.     May be responsible for discharges, assessments, and clinical documentation as needed. 

Server

This is a full-time position working three double shifts on Sunday, Monday and Wednesday and a 4- 8 evening shift. Director of Health and Wellness

Directly oversee the delivery of care to Assisted Living residents to ensure that emotional, physical, psychological and safety needs are met through all program services and activities. Coordinate, along with the Executive Director, family/resident care conferences and serve as a direct liaison between residents, families, and staff.  Work with residents, families, and other staff to ensure that residents are at appropriate levels of care.  Manage labor costs, supplies, and other line items in clinical budget.  Monitor documentation systems to ensure that all regulatory timeframes for review of assessments and service plans are met and that resident needs are met. Write and review residents’ assessments/evaluations and care-plans with appropriate staff. Assess/evaluate potential residents for Assisted Living level of care.  This may involve assessing/evaluating potential residents in their home or another healthcare setting; must be able to drive to location and perform assessment/evaluation professionally and timely if necessary. Schedule and conduct new resident orientation for all Assisted Living Residents.  Provide, plan, schedule, and monitor staff training programs and in-services for the Assisted Living staff including statutory or regulatory required training. Attend all required training, in-service, and staff meetings. Manage and coordinate the work of all nursing and other assisted living staff, communicating pertinent resident health and safety information.  This includes overseeing and/or completing staff schedules monthly and daily assignments.  Strive to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices.  Ensure compliance with all state regulatory requirements including mandatory reporting to State. Ensure that all concern and incident reports are completed, filed and that follow-up is conducted appropriately.  Assist and/or perform in mock surveys.  Maintain a positive and professional demeanor toward residents, visitors, families, and co-workers while adhering to all policies, standards and procedures of Company. Require 24/7 on-call clinical support. Perform other duties as assigned.

Utility Worker (Dishwasher)- Per Diem

Maintain sanitary condition of dishes, utensils, glasses, pots, pans, equipment and kitchen in accordance with Senior Lifestyle Corporation standards. Safely and efficiently operate dish washing machine according to procedure manual. Ensure proper soap and rinse chemicals are loaded for washing. Remove from service any dishes, utensils, and glassware that are damaged or broken. Stock delivered food and non-food items. Empty and clear garbage bins and breakdown of boxes. Deep cleaning of kitchen and equipment.     Attend all training and in-service seminars. Maintain a clean and sanitary work environment. Perform all duties in adherence to Senior Lifestyle Corporation standards. Strive to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices. Maintain a positive and professional demeanor toward all residents, visitors and co-workers. Pay Rate: $15.50-$16.50/hr

Director of Dining Services/Executive Chef

To provide local leadership and strategic direction. To oversee and control the day-to-day operation of all Dining Services operations in the community, by maintaining Senior Lifestyle Corporation standards and meeting the financial expectations of the department. This position reports to the Executive Director and may be required to supervise other staff positions. Maintains a quality assurance program that guarantees residents receive fresh, tasty, well-prepared meals, served promptly and politely. Develops well-balanced menus in collaboration with Chef. Maintains food costs on a daily basis and is responsible to ensure budgetary compliance. Responsible for purchasing all food and non-food items. Ensures that all Dining Services are stored, handled, and prepared under safe sanitary conditions as set forth by the company and the local, state, and county health departments. Acts as the facility’s representative for any health department of other required inspections; assure compliance, follow-up to assure any deficiencies are immediately corrected. Hires, trains, disciplines and terminates departmental employees in accordance with Senior Lifestyle Corporation human resources policy. Oversees the scheduling of all food service personnel to maintain compliance with the budgetary guideline. Assures all department training standards are met and up to date. Maintains all department logs and ensures bookkeeping policies are followed. Establish daily interaction with residents. Orders all dining room supplies. Employs appropriate loss prevention procedures. Ensures all Dining Services employees adhere to the grooming and uniform policies. Works with appropriate staff to coordinate special events. Performs all duties in adherence to Senior Lifestyle Corporation standards. Strives to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices. Participates in Manger on Duty program. Maintains a positive and professional demeanor toward all residents, visitors, and co-workers.
