Registered Nurse – Skilled Nursing

Responsible for medication supervision/administration in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines. May be responsible for the admissions of new residents to include all admission assessments and documentation. Responsible for completing acute and long-term care plans or service plans.  May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the Director of Nursing. May be responsible for discharges, assessments, and clinical documentation as indicated.  Provide general consultation regarding health-related concerns within the community. Notify physicians and/or family members of any change in resident’s health and provides proper documentation. Respond quickly to all emergencies. Maintain close communication with all departments and staff members. Maintain confidentiality of residents’ and prospective residents’ information. Understand role in the Safety & Disaster Plan. Provide educational meetings for residents and staff.

Resident Program Assistant-PRN

Assist in planning and coordinating all resident activities.  Encourage residents to participate in programs and activities and provide transportation to them as needed. Lead various activities as directed by the Resident Programs Director. Order and maintain supplies for activities as needed.

Licensed Practical Nurse (LPN) – Skilled Nursing- FULL TIME DAYS

Certified Nursing Assistant (CNA) – Assisted Living – AM shift – FT

Take and record vital signs. Measure and record height, weight, and fluid intake/output. Recognize abnormal changes in body functioning and importance of reporting such changes to a supervisor. Assist as needed with medication reminders, bathing, grooming, dressing, escort service, and other activities of daily living. Transfer, position, and turn residents. Provide skin care to residents. Contribute to the resident’s assessment and the plan of care. Provide nursing rehabilitation/restorative nursing services. Follow the schedule of resident’s needs set out by supervisor. Provide emotional and social support to residents. Promote and protect resident rights, assist residents in making informed decisions, treat residents with dignity and respect, protect resident’s personal belongings, report suspected abuse or neglect, avoid the need for physical restraints in accordance with the current professional standards, and support independent expression, choice, and decision-making consistent with applicable law and regulation. Inform supervisor of any resident issues or concerns. Respect and encourage the independence and dignity of the residents. Respect residents’ confidentiality.

Resident Aide – Part-Time – 1st Shift – Starting Wage $16

Provide assistance as needed with medication reminders, bathing, grooming, dressing, escort service, and other activities of daily living. Follow the schedule of resident’s needs set out by supervisor. Provide emotional and social support to residents. Document daily log of assistance. Inform supervisor of any resident issues or concerns. Respect and encourage the independence and dignity of the residents. Respect residents’ confidentiality.

Community Relations Director

Pay Rate: $77,000/year Schedule, organize, and conduct tours with prospective resident of independent and assisted living, memory care and skilled nursing. Respond and follow-up with to all walk-ins, phone ins, mail-ins timely and appropriately. Create, plan and implement with Sales and Marketing Team and Program Director events to bring in prospective residents, current leads and professionals such as, but not limited to: seminars, speakers, themed events, and family nights. Develop and maintain a good working relationship with residents, families, and professional providers of care.    Maintain a working knowledge of all software programs. Participate in weekend call coverage in Manager Rotation unless they have a standing schedule that already encompasses one or more weekend day.  Secure the required number of budgeted move ins and achieve net gains each month Treat each inquiry with value. Work closely with the Executive Director and Regional Director of Operations, and Divisional Director of Sales and Marketing in development and implementation of the quarterly marketing plan. Create and implement a strong community outreach program that generate the necessary number of referrals to reach sales goals. Complete quarterly media planning with current vendor on a timely basis.  Keep Executive Director informed of all sales activity daily. Maintain Senior Lifestyle Hot Board/Move in Move out board. Communicate any special needs of the incoming resident to the appropriate personnel. Manage use of sales toolbox. Assure every alternative is considered before closing a lead.  Keep informed of all trends, developments, concepts and techniques in his/her field that affect product. Understand the need of the senior and the aging process. Maintain and work an active, meaningful waitlist.  Visit competitors quarterly. Maintain confidentiality of all pertinent information. Comply with all Senior Lifestyle Policies, cities, state, and federal regulations pertaining to Residents’ accounts as it relates to Medicare, third party billing, etc. Interact professionally and effectively with all levels of the organization, residents, family members, etc. Perform other duties as assigned.
 

Multi-level Nursing Home Administrator (MLNHA)

Act as the on-site executive for all operations, including being the main point of contact for all staff, residents, prospects, community organizations, government agencies and the public when needed.  Maintains resident retention by working with residents and their families, dealing with any issues that arise. Responds and follows up with all walk-ins, phone-ins, mail-ins timely and appropriately. Keeps up to date with information about competitors. Meets all expectations of meaningful contacts, leases and occupancy. Maintain budget accountability and cash flow; aggressively anticipate and minimize negative budget variances and deficits. Hire, train, discipline, and terminate employees in accordance with Senior Lifestyle Corporate policy.  Maintain all local, state, and federal licenses for the community. Lead staff meetings.  Promote and protect resident rights; assisting residents to make informed decisions and treating them with dignity and respect.  Become an intricate part of the community in social and civic affairs by representing the community in local, state, and professional organizations.  Manage other support level and management roles as needed. 

10pm-6am PRN Care Associate – Assisted Living (AL)

Provide assistance as needed with medication reminders, bathing, grooming, dressing, escort service, and other activities of daily living. Follow the schedule of resident’s needs set out by supervisor. Provide emotional and social support to residents. Document daily log of assistance. Inform supervisor of any resident issues or concerns. Respect and encourage the independence and dignity of the residents. Respect residents’ confidentiality.

Dietary Manager for our Skilled Community. CDM required

Supervise daily operation of dining room(s), room service and convenience store. Ensure room service orders are delivered timely and properly. Ensure smooth and timely opening of the dining room. Assist in producing weekly schedules with budgetary guidelines for service staff. Ensure that there are an adequate number of employees for each shift and ensure that absences are covered. Lead the training of all new dining room employees. Assist service staff during mealtime as needed. Oversee resident billing of food service charges. Manage dining staff.
