Resident Program Coordinator

Plan, develop, and implement resident lifestyle programming within the community as well as through outside trips and events. Survey and interview residents to determine their past and present interests and talents, as well as their satisfaction with the community’s programs and activities. Recognize the individuality, diversity, and spirituality of all residents when planning activities and celebrating holiday functions. Develop relationships with residents to encourage their participation in activities that enhance their quality of and appreciation for life. Observe residents’ function and behavior and reports significant changes or concerns to appropriate staff. Utilize outside community resources and independent contractors to enrich the variety of programs made available to residents. Assist with developing and managing the annual department budget.   Assist with managing Resident Program Assistants as needed in the absence of the Director of Resident Programs. 

Concierge – Part Time

Greet residents and provide general assistance to visitors, prospective residents, and families. Answer telephone calls. You may be asked to ensure the safety of the residents and property by monitoring surveillance cameras, door alarms, life safety systems, building alarms and investigating any unusual activity. Process reports for such things as work orders, incidents and concerns. You will receive packages, plants, prescription drugs, etc. and deliver or hold as requested.  Maintain the sign in/out log for the issuance of community keys, and gate passes. You will assist other departments with administrative functions. Ensure that resident/family/employee concerns receive timely response and follow up, thus striving to provide a high level of customer satisfaction. Maintain a positive and professional demeanor toward all residents, visitors and co-workers.

Director of Dining Services

Follow Company Dining Policy and Procedures and while ensuring that all department personnel are compliant as well.  Responsible for the purchasing of all dining food and nonfood items and ensure all purchases for the department achieve 95% DSSI ordering compliance.  Balance and maintain budgets for raw food and supplies. Utilize company standard recipes when completing food production for meals. Ensure that food is served at proper temperature and in proper portion sizes to ensure food safety and palatability according to our food temperature policy.   Ensure that residents’ Prescribed Diets are offered when applicable. Drive high satisfaction by offering our residents choice, dignity and independence in dining according to our hospitality and customer service policy.  Ensure operating and cleaning instructions/procedures are developed and implemented for the department.  Perform Pre-meal Dining Stand-Up with service staff to review daily menu details. Know and adhere to all company and the local, state, and county health departments regulations and acts as the community’s dietary representative for any health department or other required inspections; assure compliance, follow-up to assure any deficiencies are immediately corrected. Ensure all dining areas are clean and safely operating, completes monthly Self Sanitation audit according to the sanitation audit tool and operation and sanitation policies.   Interview, manage, train, hire, discipline, and terminate all dining staff personnel as needed. Oversee the scheduling of all food service personnel to maintain compliance with labor budget.

**Multi-Level Administrator License Needed** Act as the on-site executive for all operations, including being the main point of contact for all staff, residents, prospects, community organizations, government agencies and the public when needed.  Maintains resident retention by working with residents and their families, dealing with any issues that arise. Responds and follows up with all walk-ins, phone-ins, mail-ins timely and appropriately. Keeps up to date with information about competitors. Meets all expectations of meaningful contacts, leases and occupancy. Maintain budget accountability and cash flow; aggressively anticipate and minimize negative budget variances and deficits. Hire, train, discipline, and terminate employees in accordance with Senior Lifestyle Corporate policy.  Maintain all local, state, and federal licenses for the community. Lead staff meetings.  Promote and protect resident rights; assisting residents to make informed decisions and treating them with dignity and respect.  Become an intricate part of the community in social and civic affairs by representing the community in local, state, and professional organizations.  Manage other support level and management roles as needed. 

Nursing Home Administrator

Sous Chef

Prepare meals from standardized recipes in accordance with order in a timely and attractive manner. Set up food service line and workstations with needed condiments. Ensure all condiments and garnishes that are required are prepared and served with the appropriate foods. Assist in coordinating prep for the following day’s menu. Participate in a pre-meal meeting with food servers to review detail of daily menu.   Assist with training of new kitchen employees. Assist with scheduling of employees within the department.  Assist with food ordering, monitoring inventory and creating the menu cycle. Keep stock rooms, coolers, and freezers clean and ensure that food supply stocks are rotated and that all perishables are labeled, dated, and stored properly. Maintain kitchen cleanliness and food preparation according to state and local health department code requirements. Keep food waste to a minimum by utilizing food storage and food recycling techniques. Manage and supervise Cooks, Lead Cooks, Prep Cooks, and Utility Workers as needed.

Director of Nursing

Ensure, with assistance of the Staffing Coordinator, that appropriate and adequate staffing is in place on all nursing units, all shifts, every day. Complete and maintain all reports, i.e. I/A logs, monthly QA log, e-Interact, etc. and submit timely to corporative representative as required. Chair/attend meetings to include the weekly At Risk, weekly MC, care plan as per invitation, daily morning stand-up and clinical, etc. Consult with the ADON and Charge Nurse/Unit Manager to identify, investigative, educate, and discipline nurses related to care or skill issues as indicated. Oversee the clinical processes/systems in the facility to ensure that practice is maintained as per Senior Lifestyle standards, policies & procedures and within state and federal regulatory guidelines. Manage the survey process when present in the facility. Coordinate with the Admissions Coordinator to review all potential admissions identifying potential care, behavioral, family issues and the facility’s staff ability to care for. Monitor current health records, follow-up on incidents and issues with skilled residents and/or families, and schedule meetings as needed. Follow-up as indicated with falls, infection control, skin issues, unusual occurrences, medication delivery error, and any other issues requiring the attention of a senior administrative member. Ensure, review, and follow-up with quality assurance parameters including chart audits, log audits, and the scheduling of in-services as indicated. Develop and maintain a working relationship with professional providers of care to skilled residents both within and outside the facility. Maintain a professional and informative relationship with both the NHA and ED of the community, keeping them abreast of nursing issues as they develop and any potential impact on the facility.

Certified Nursing Assistant (CNA) – Skilled Nursing

Take and record vital signs. Measure and record height, weight, and fluid intake/output. Recognize abnormal changes in body functioning and importance of reporting such changes to a supervisor. Assist as needed with medication reminders, bathing, grooming, dressing, escort service, and other activities of daily living. Transfer, position, and turn residents. Provide skin care to residents. Contribute to the resident’s assessment and the plan of care. Provide nursing rehabilitation/restorative nursing services. Follow the schedule of resident’s needs set out by supervisor. Provide emotional and social support to residents. Promote and protect resident rights, assist residents in making informed decisions, treat residents with dignity and respect, protect resident’s personal belongings, report suspected abuse or neglect, avoid the need for physical restraints in accordance with the current professional standards, and support independent expression, choice, and decision-making consistent with applicable law and regulation. Inform supervisor of any resident issues or concerns. Respect and encourage the independence and dignity of the residents. Respect residents’ confidentiality.

Certified Medication Aide – Assisted Living (AL)

Responsible for supervision of self-administered medication for the Assisted Living Department in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines as directed by the LPN/LVN/RN and/or the Assisted Living Director/Health and Wellness Director. Count and sign on approved form, all narcotics with approved personnel before accepting medication keys.  Contact a supervisor immediately with discrepancies. Order all necessary medication and treatments from appropriate pharmacy or notify responsible party when supply is no less than 7 days. Notify supervisor when supply is less than 2 days and medicine hasn’t been received. Responsible for receiving and verifying all medicine received is accurate according to pharmacy label and MAR.  Notifies pharmacy and DH&W of any discrepancies. Responsible for proper storage of all medications and ensuring all lotions, ointments and liquids stored separately from oral medications. Responsible for dating all appropriate vials or bottles at time of initial opening and following pharmacy protocol on length of time they can be used. Responsible for weekly inspections of all medications to ensure all expired medications are removed. Responsible for verifying all current MARs are compared to new MARs at change over time and notifying the supervisor of any discrepancies. Responsible for removing all discontinued medication from med cart and giving it to the supervisor. Provide personal care to residents to include, but not limited to, bathing, skin care, oral hygiene, grooming, dressing, incontinence care, and eating, etc. Provide general nursing care to include, but not limited to, vital signs, temperatures, blood pressure, and weights. Document appropriate information in the daily log as required and notify supervisor of all concerns. Answer calls and assist residents with needs as assigned. Responsible for daily cleaning of all medicine carts at end of shift and restocking as needed.  Responsible for weekly deep cleaning of all med carts. Attend and participate in staff training and in-services. Must maintain accurate records to include preparation of all medical charts for new admissions and readmissions and ensures medical record is complete within forty-eight (48) hours as Policy and Procedure (P&P) and regulatory guidelines.
