Housekeeper

General cleaning of resident apartments, hallways and other common areas. Assist in preparing empty apartments for resident move-ins. Ensure resident/guest satisfaction by responding quickly to needs and requests.  You may be asked to help respond to resident emergencies. Licensed Practical Nurse (LPN) – Skilled Nursing

Responsible for medication administration for the residents in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines. May be responsible for the admissions of new residents to include all admission assessments and documentation. Responsible for completing acute and long-term care plans or service plans. May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the director. Provide general consultation regarding health-related concerns within the community. Notify physicians and/or family members of any change in resident’s health and provides proper documentation. Respond quickly to all emergencies. Maintain close communication with all departments and staff members. Provide educational meetings for residents and staff. Understand roll in the Safety & Disaster Plan. Maintain confidentiality of residents’ and prospective residents’ information.     May be responsible for discharges, assessments, and clinical documentation as needed. 

Assistant Director of Nursing

Responsible for the proper maintenance of medical records to include monitoring documentation in Point Click Care.  Monitor current health records, follow-up on incidents, issues with skilled residents and/or families, and meetings as needed. Develop and maintain a working relationship with professional providers of care to skilled residents.  Assist the direct care nursing staff with admissions, transfers, and the discharge of residents. Monitor and follow-up with falls, infection control, skin issues, unusual occurrences, medication delivery error and any other issues as indicated by the DON.  Maintenance of quality assurance parameters including chart audits, log audits and the scheduling of in-services as indicated. Oversee the daily completion and adherence to documentation as required in PCC. Complete audits as per direction of the DON. Assist the direct care staff with admissions, transfers & discharges. Provide direct care nursing and supervisor responsibilities as indicted. Provide staff in-service and education as indicated. Communicate, clarify and obtain orders, and updates on resident condition, etc. with other Health Care Providers.

MDS Coordinator – Skilled Nursing

Coordinate the assessment schedule and opens resident assessments in accordance with current Federal and state rules, regulations and guidelines that govern the resident assessment instrument, including the implementation of CAAs and CATS. Select the Assessment Reference Date (ARD) that will most accurately reflect the resident’s condition based upon the capture of all resources utilized and care provided.  Gather the necessary documentation to accurately encode the assessment item set and ensure that it is complete, accurate, signed and submitted to the state database in accordance with RAI or state regulations. Meet daily with the Rehab Program Manager to review Resident therapy schedules in order to minimize the need for non-scheduled assessments. Coordinate with the interdisciplinary team to ensure the care plan is comprehensive, completed timely, and remains current based on the Resident’s condition.  Attend and participate in the Resident Care Conference. Disseminate any new or updated materials involving the RAI process to the interdisciplinary team to include assessment calendars, due dates, etc. Facilitate the involvement of appropriate health professionals as needed to improve or maintain the Resident’s functional abilities at the highest practicable level.  Assist and/or direct Managed Care case management duties. Review the state validation reports and ensure that the appropriate follow-up action is taken, if needed. Serve on, participate in, and attend various other committees of the facility (e.g. At Risk, Triple Check, Medicare UR) as required, and as directed by the Administrator or Executive Director. Communicate with and provide education to all nurses and CNAs to expand their knowledge of the RAI process and how their duties can impact the process.   Communicate with Business Office regarding unfinished assessments, defaults, and other billing concerns. Understand, comply with, and promote all rules and regulations regarding Residents’ Rights; promote positive relationships with Residents, visitors, and regulators, to include presenting a professional appearance.  Maintain HIPAA compliance at all times.

Assistant Director of Plant Operations

The Assistant Plant Operations Director is responsible assisting the Director of Plant Operations in the maintenance and upkeep of the physical plant including, building, grounds and all Maintenance.

Driver / Transportation

Transport residents to appointments and perform community errands as assigned.  Ensure that all residents who use the transportation are returned safely to the building. Assist all residents in and out of the vehicle as needed.  Maintain cleanliness of vehicle by performing and/or scheduling regular interior/exterior cleaning. Report mechanical needs and repairs and assist in coordination to have repairs completed.

Certified Nursing Assistant (CNA) – Part Time

Take and record vital signs. Measure and record height, weight, and fluid intake/output. Recognize abnormal changes in body functioning and importance of reporting such changes to a supervisor. Assist as needed with medication reminders, bathing, grooming, dressing, escort service, and other activities of daily living. Transfer, position, and turn residents. Provide skin care to residents. Contribute to the resident’s assessment and the plan of care. Provide nursing rehabilitation/restorative nursing services. Follow the schedule of resident’s needs set out by supervisor. Provide emotional and social support to residents. Promote and protect resident rights, assist residents in making informed decisions, treat residents with dignity and respect, protect resident’s personal belongings, report suspected abuse or neglect, avoid the need for physical restraints in accordance with the current professional standards, and support independent expression, choice, and decision-making consistent with applicable law and regulation. Inform supervisor of any resident issues or concerns. Respect and encourage the independence and dignity of the residents. Respect residents’ confidentiality.

Social Worker Full time

As part of the admission process, meet with the resident and family to review policies on advanced directives, resident rights, and discrimination. Conduct and document a social services evaluation, including identification of Resident problems/needs. Provide/arrange for social work services as indicate by resident/family needs. Assist residents, as needed, with financial planning. Keep the resident’s family informed of resident problems, personal needs, transfers, and changes of level assignment. Maintain a current list of community resources to facilitate referrals. Working knowledge and ability to apply professional standards of social work practice in job situations. Able to establish criteria to assure that services provided meet established standards of quality. Demonstrate satisfactory level of interpersonal skills to interact with residents, families, community and interdisciplinary staff. Demonstrate assessment skills sufficient to evaluate residents’ behavior, to collect data to evaluate psycho- social needs, risk factors for psycho- social deterioration and residents’ responses to interventions. Able to interpret and explain resident behavior to staff in way that fosters understanding, facilitates treatment and respect for resident rights and minimizes use of chemical and physical restraints. Implement social service interventions that achieve treatment goals, address resident needs, link social supports, physical care and physical environment to enhance quality of life.

CNA in Training – “Earn while you learn” Class starts May 13th.

Attends all classes and clinical assignments.  Adheres to course attendance requirements.  Understands and adheres to student camera use requirement during online courses. Provides care to assigned residents by demonstrating clinical skill mastery and clinical knowledge obtained through classes and assignments.  under the supervision of a clinical instructor. Strive to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices. Maintain a positive and professional demeanor toward residents, visitors, families, and co-workers. Adhere to all policies and procedures of Senior Lifestyle Corporation. Perform other duties as assigned. Upon completion of the CNA training program, demonstrate the ability/knowledge to execute the following skills: Take and record vital signs. Familiar with emergency equipment and procedures. Understand duties during a fire drill and is familiar with evacuation plan.     Attend all required training, in-service, and staff meetings.
