Community Liaison FT (Salaried)

Conduct outreach geared towards skilled nursing and rehab market and other levels of care as requested. Responsible for screening potential admissions for eligibility. Handle entire admission and admission related tasks/issues. Process admission paperwork with resident/family.  Schedule, organize, and conduct tours with prospective residents and referral sources for skilled nursing. Respond and follow-up with all walk-ins, phone ins, mail-ins timely and appropriately. Complete daily report for skilled nursing activity for admissions, discharges, outreach and other tasks as needed. Keep Sales and Marketing Director and Skilled Nursing Administrator informed of all sales activity daily. Provide assistance to the sales and marketing director at independent/assisted/ memory care campuses and special assigned projects. Create, plan, and implement with Sales and Marketing Team and Program Director events to bring in prospective residents such as but not limited to: seminars, speakers, themed events, and family nights. Develop and maintain a good working relationship with residents, families and professional providers of care.    Maintain a working knowledge of all software programs. Coordinate admissions/transfers from various levels of care to/from skilled nursing and rehab. Work with AL/MC Managers to assist with moving resident’s through the continuum of care. Meet all expectations of occupancy. Participate in weekend call coverage in Manager Rotation.   Responsible for marketing budget for skilled nursing.

Valet/Concierge FT (Day/PM shift Sunday-Thursday) ***$17.50/hour***

Answer calls at front desk. Place work orders as needed. Deliver packages to residents as needed. Assist with maintaining the mailroom. Make sure front of building is clear of debris. Cover breaks at front desk. Drive vehicle to and from parked area, secure the vehicle and note specific information on the ticket. Organize, monitor, and keep track of the tags and keys at the valet booth. Greet all guests upon arrival, open doors, explain valet procedures and pricing. Assist guests if they need directions.  Perform access control responsibilities for the valet area.

Licensed Practical Nurse (LPN) PT (AM: 7am-3pm) or (PM: 3pm-11pm)

Responsible for medication administration for the residents in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines.  May be responsible for the admissions of new residents to include all admission assessments and documentation.  Responsible for completing acute and long-term care plans or service plans.  May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the director.  Provide general consultation regarding health-related concerns within the community.  Notify physicians and/or family members of any change in resident’s health and provides proper documentation.  Respond quickly to all emergencies.  Maintain close communication with all departments and staff members.  Provide educational meetings for residents and staff.  Maintain confidentiality of residents’ and prospective residents’ information.  Attend all required training, in-service, and staff meetings.  Strive to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices.

Assistant Director of Nursing

Responsible for the proper maintenance of medical records to include monitoring documentation in Point Click Care.  Monitor current health records, and follow-up on incidents and issues with skilled residents and/or families. Develop and maintain a working relationship with professional providers of care to skilled residents.  Assist the direct care nursing staff with admissions, transfers, and the discharge of residents. Complete and log chart audits.  Monitor and follow-up with falls, infection control, skin issues, unusual occurrences, medication delivery error. Oversee the daily completion and adherence to required documentation. Provide direct care nursing as needed. Provide staff in-service and education as needed. Manage licensed and non-licensed care staff as needed.

Assisted Living Attendant/Caregiver

Provide assistance as needed with medication reminders (as applicable), bathing, grooming, dressing, escort service, and other activities of daily living. Provide emotional and social support to residents. Document daily log of assistance. Respect and encourage the independence and dignity of the residents. Respect residents’ confidentiality. Ensure that resident/family/employee concerns receive timely response and follow up, thus striving to provide a high level of customer satisfaction. Maintain a positive and professional demeanor toward all residents, visitors and co-workers.

Registered Nurse (RN)

Responsible for medication supervision/administration for the Assisted Living Department in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines. Provide general consultation regarding health-related concerns within the community. Notify physicians and/or family members of any change in resident’s health and provides proper documentation. Respond quickly to all emergencies. Maintain close communication with all departments and staff members. Maintain confidentiality of residents’ and prospective residents’ information. Provide educational meetings for residents and staff.

Certified Nursing Assistant (CNA) – Assisted Living FT – AM shift (6 AM-2 PM) or PM shift (2 PM-10 PM) or Night shift (10 PM-6 AM)

Take and record vital signs. Measure and record height, weight, and fluid intake/output. Recognize abnormal changes in body functioning and importance of reporting such changes to a supervisor. Assist as needed with medication reminders, bathing, grooming, dressing, escort service, and other activities of daily living. Transfer, position, and turn residents. Provide skin care. Contribute to the resident’s assessment and the plan of care. Provide nursing rehabilitation/restorative nursing services. Care for residents with dementia. Follow the schedule of resident’s needs set out by supervisor. Provide emotional and social support to residents. Promote and protect resident rights, assist residents to make informed decisions, treat residents with dignity and respect, protect resident’s personal belongings, report suspected abuse or neglect, avoid the need for physical restraints in accordance with the current professional standards, and support independent expression, choice, and decision-making consistent with applicable law and regulation. Inform supervisor of any resident issues or concerns. Respect and encourage the independence and dignity of the residents. Respect residents’ confidentiality. Familiar with emergency equipment and procedures. Understand duties during a fire drill and is familiar with evacuation plan.     Attend all required training, in-service, and staff meetings. Strive to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices. Maintain a positive and professional demeanor toward residents, visitors, families, and co-workers. Adhere to all policies and procedures of Senior Lifestyle Corporation.
