Starting Pay $40-$44. Responsible for the proper maintenance of medical records to include monitoring documentation in Yardi EHR.  Monitor current health records, follow-up on incidents, issues with skilled residents and/or families, and meetings as needed. Develop and maintain a working relationship with professional providers of care to skilled residents.  Assist the direct care nursing staff with admissions, transfers, and the discharge of residents. Monitor and follow-up with falls, infection control, skin issues, unusual occurrences, medication delivery error and any other issues as indicated by the HWD.  Maintenance of quality assurance parameters including chart audits, log audits and the scheduling of in-services as indicated. Oversee the daily completion and adherence to documentation as required in Yardi EHR. Complete audits as per direction of the HWD. Assist the direct care staff with admissions, transfers & discharges. Provide direct care nursing and supervisor responsibilities as indicted. Provide staff in-service and education as indicated. Communicate, clarify and obtain orders, and updates on resident condition, etc. with other Health Care Providers.

Provide general consultation regarding health-related concerns within the community. Notify physicians and/or family members of any change in resident’s health and provides proper documentation. Respond quickly to all emergencies. Maintain close communication with all departments and staff members. Provide educational meetings for residents and staff. Maintain confidentiality of residents’ and prospective residents’ information.     May be responsible for discharges, assessments, and clinical documentation as needed.

$17/hr Accept food order, communicate to kitchen and deliver food and beverage to residents and/or guests in a timely manner. Ensure resident/guest satisfaction by responding quickly to needs and requests.  Actively buss and reset tables as well as complete assigned side work. Maintain a positive and professional demeanor toward all residents, visitors and co-workers.

Seeking Part-time weekend/ PRN team members to join, with flexible hours available! $19-$22/HR  Provide assistance as needed with medication reminders, bathing, grooming, dressing, escort service, and other activities of daily living. Follow the schedule of resident’s needs set out by supervisor. Provide emotional and social support to residents. Document daily log of assistance. Respect and encourage the independence and dignity of the residents. Respect residents’ confidentiality. Ensure that resident/family/employee concerns receive timely response and follow up, thus striving to provide a high level of customer satisfaction. Maintain a positive and professional demeanor toward all residents, visitors and co-workers. Assist with resident’s laundry and meal service.

$18/HR Maintain sanitary condition of dishes, utensils, glasses, pots, pans, equipment and kitchen in accordance with Senior Lifestyle Corporation standards. Safely and efficiently operate dish washing machine according to procedure manual. Ensure proper soap and rinse chemicals are loaded for washing. Remove from service any dishes, utensils, and glassware that are damaged or broken. Stock delivered food and non-food items. Empty and clear garbage bins and breakdown of boxes. Deep cleaning of kitchen and equipment.     Attend all training and in-service seminars. Maintain a clean and sanitary work environment. Perform all duties in adherence to Senior Lifestyle Corporation standards. Strive to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices. Maintain a positive and professional demeanor toward all residents, visitors and co-workers.

$23/hr  Sunday-Thursday 5am-1:30pm Prepare all food using the daily planner and SLC standard recipes. Work the food line during mealtimes. Ensure that all food meets safety guidelines and that all kitchen areas meet sanitation guidelines. Ensure resident and guest satisfaction by responding quickly to needs and requests.  Maintain a positive and professional demeanor toward all residents, visitors and co-workers.
