Housekeeper

Director of Dining Services / Executive Chef

Salary $85k We are seeking an experienced and visionary Director of Dining Services / Executive Chef to lead our culinary operations in Albuquerque. In this pivotal role, you will oversee all aspects of our dining program, from strategic planning and menu innovation to operational excellence and team leadership. The ideal candidate will combine exceptional culinary expertise with strong business acumen and a passion for delivering outstanding dining experiences. Server (Full-Time with Benefits)

Accept food order, communicate to kitchen and deliver food and beverage to residents and/or guests in a timely manner. Ensure resident/guest satisfaction by responding quickly to needs and requests.  Actively buss and reset tables as well as complete assigned side work. Care Associate – Memory Care (MC)

Salary Range $17-$18 per hour  Provide kind, caring, and consistent companionship and support to residents. Provide assistance, encouragement, and restorative interventions, as needed, for Memory Care residents to be as independent as possible in their activities of daily living including but not limited to: bathing, dressing, grooming, toileting, feeding, and mobility. Participate in and lead one on one and group activities. Serve residents during mealtime, assist with memory care laundry, and basic housekeeping within the memory care neighborhood. Ensure that resident/family/employee concerns receive timely response and follow up, thus striving to provide a high level of customer satisfaction. Maintain a positive and professional demeanor toward all residents, visitors and co-workers.

Care Associate – Assisted Living (AL)

Salary $17 – $18 per hour   We are looking for second shift Team Members  (2 pm-10 pm) both Full 
Time and Part Time Provide assistance as needed with medication reminders, bathing, grooming, dressing, escort service, and other activities of daily living. Follow the schedule of resident’s needs set out by supervisor. Provide emotional and social support to residents. Document daily log of assistance. Inform supervisor of any resident issues or concerns. Respect and encourage the independence and dignity of the residents. Respect residents’ confidentiality.

Certified Medication Aide – Assisted Living (AL)

Salary starting at $17 per hour based on experience Responsible for supervision of self-administered medication for the Assisted Living Department in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines as directed by the LPN/LVN/RN and/or the Assisted Living Director/Health and Wellness Director. Count and sign on the approved form, all narcotics with approved personnel, before accepting medication keys.  Contact a supervisor immediately with discrepancies. Order all necessary medication and treatments from the appropriate pharmacy or notify the responsible party when the supply is no longer than 7 days. Notify the supervisor when the supply is less than 2 days, and the medicine hasn’t been received. Responsible for receiving and verifying that all medicine received is accurate according to the pharmacy label and MAR: Notifies the pharmacy and DH&W of any discrepancies. Responsible for proper storage of all medications and ensuring all lotions, ointments, and liquids are stored separately from oral medications. Responsible for dating all appropriate vials or bottles at the time of initial opening and following pharmacy protocol on the length of time they can be used. Responsible for weekly inspections of all medications to ensure all expired medications are removed. Responsible for verifying that all current MARs are compared to new MARs at the changeover time and notifying the supervisor of any discrepancies. Responsible for removing all discontinued medication from the med cart and giving it to the supervisor. Provide personal care to residents to include, but not limited to, bathing, skin care, oral hygiene, grooming, dressing, incontinence care, and eating, etc. Provide general nursing care to include, but not limited to, vital signs, temperatures, blood pressure, and weights. Document appropriate information in the daily log as required and notify the supervisor of all concerns. Answer calls and assist residents with needs as assigned. Responsible for daily cleaning of all medicine carts at the end of the shift and restocking as needed.  Responsible for weekly deep cleaning of all med carts. Attend and participate in staff training and in-services. Must maintain accurate records to include preparation of all medical charts for new admissions and readmissions and ensures medical record is complete within forty-eight (48) hours as Policy and Procedure (P&P) and regulatory guidelines.

Cook

Salary $18 per hour FT with alternating Weekends.  Interview Includes a Cooking Demo  Prepare all food using the daily planner and SLC standard recipes. Work the food line during mealtimes. Ensure that all food meets safety guidelines and that all kitchen areas meet sanitation guidelines. Ensure resident and guest satisfaction by responding quickly to needs and requests.  Maintain a positive and professional demeanor toward all residents, visitors and co-workers.

Director of Health and Wellness

Salary $90k – $100k (Plus a $5k Sign On Bonus!) Directly oversee the delivery of care to Assisted Living residents to ensure that emotional, physical, psychological and safety needs are met through all program services and activities. Coordinate, along with the Executive Director, family/resident care conferences and serve as a direct liaison between residents, families, and staff.  Work with residents, families, and other staff to ensure that residents are at appropriate levels of care.  Manage labor costs, supplies, and other line items in clinical budget.  Monitor documentation systems to ensure that all regulatory timeframes for review of assessments and service plans are met and that resident needs are met. Write and review residents’ assessments/evaluations and care-plans with appropriate staff. Assess/evaluate potential residents for Assisted Living level of care.  This may involve assessing/evaluating potential residents in their home or another healthcare setting; must be able to drive to location and perform assessment/evaluation professionally and timely if necessary. Schedule and conduct new resident orientation for all Assisted Living Residents.  Provide, plan, schedule, and monitor staff training programs and in-services for the Assisted Living staff including statutory or regulatory required training. Attend all required training, in-service, and staff meetings. Manage and coordinate the work of all nursing and other assisted living staff, communicating pertinent resident health and safety information.  This includes overseeing and/or completing staff schedules monthly and daily assignments.  Strive to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices.  Ensure compliance with all state regulatory requirements including mandatory reporting to State. Ensure that all concern and incident reports are completed, filed and that follow-up is conducted appropriately.  Assist and/or perform in mock surveys.  Maintain a positive and professional demeanor toward residents, visitors, families, and co-workers while adhering to all policies, standards and procedures of Company. Require 24/7 on-call clinical support. Perform other duties as assigned.

Licensed Practical Nurse (LPN) – Assisted Living

Salary $37 per hour – Full Time Responsible for medication administration for the residents in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines. May be responsible for the admissions of new residents to include all admission assessments and documentation. Responsible for completing acute and long-term care plans or service plans. May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the director. Provide general consultation regarding health-related concerns within the community. Notify physicians and/or family members of any change in resident’s health and provides proper documentation. Respond quickly to all emergencies. Maintain close communication with all departments and staff members. Provide educational meetings for residents and staff. Maintain confidentiality of residents’ and prospective residents’ information.     May be responsible for discharges, assessments, and clinical documentation as needed.
