Memory Care Coordinator

Promote the company’s Embrace philosophy in all aspects of his/her role for residents, families and staff. Implement and oversee the six unique pillars of Embrace. Daily oversight of the operations in the Memory Care area in partnership with the Memory Care Director and Health and Wellness team.  Lead high-quality innovative programs in Memory Care. This is the positions primary focus. Person will personally lead a minimum of 5-7 programs per day. Assist in planning, coordinating, and evaluating resident activities. Encourage resident participation in programs. Provide education, training, and hands on support to community staff in partnership with the community managers. Implement and monitor programs and process to promote excellence in programming while maintaining a high level of resident/family/staff satisfaction in a fiscally responsible manner.

Director of Social Services

Working knowledge and ability to apply professional standard of social work practice in job situations.  Demonstrate satisfactory level of interpersonal skills to interact with residents, families, community, government agencies, and interdisciplinary staff. Able to establish criteria to assure that services provided meet established standards of quality.  Demonstrate assessment skills sufficient to evaluate resident’s behavior, to collect data to evaluate psychosocial needs, risk factors for psychosocial deterioration, and residents’ responses to interventions. Ability to interpret and explain resident behavior to staff in a way that fosters understanding, facilitates treatment, and respect for residents rights. Minimize the use of chemical and physical restraints.  Ability to implement social services interventions that achieve treatment goals, address resident needs, link social support, physical care, and physical environment to enhance quality of life.  Promote and protect resident rights, assist residents to make informed decisions, treat residents with dignity and respect. Protect residents’ personal belongings, report suspected abuse or neglect, avoid the need for physical restraints in accordance with the current professional standards. Support independent expression, choice, and decision-making consistent with applicable law and regulation.  Complete progress notes and track/complete PASRRS.

Manager Nurse – Assisted Living

Provide direct and individualized nursing care to assigned residents based on established nursing protocol. Observe and assess for: pain/discomfort, oxygen therapy, all drainage, all dressings, and any incontinent elimination which may have occurred.  Provide ignition and ongoing systematic assessment of the physical, emotional, social, educational, and functional needs of the resident/family through observation, interview, and physical examination.  Interpret assessment data and information to assist in developing a plan of care.  Assure medical orders are transcribed and processed accurately and timely and integrate the medical care plan into the provision of nursing care. Assure that medication administration is correct, according to the established nursing policies and procedures. Provide accurate and timely documentation consistent with the resident plan of care.  Administer medication per physician orders. Document properly on MOR. Observe for any adverse reaction.  Assemble resident charts, resident records, and assure all necessary form are completed in a timely manner. Assure all monthly summaries are completed in a timely manner for assigned residents.  Prepare incident/accident reports for residents as needed per policy, notify AL Director, physician, and family post incident per policy and document accordingly.  Assist resident in learning self-care activities.  Supervise all resident assignments to nursing team members, and assure special instructions are given to team members.  Observe and supervise all meals to ensure residents receive the proper diet and any assistance required.  Monitor and organize all material resources necessary for meeting needs of assigned unit.  Recognize emergencies and life-threatening situations and initiate appropriate action. Ensure that resident call lights are promptly answered, and their needs are addressed timely and appropriately.  Assist with the orientation of new staff members.  Ensure that all scheduled activities take place on assigned unit by utilizing activity personnel and/or nursing personnel. 

Housekeeping Supervisor

Assist in coordinating and staffing schedules to ensure the cleaning of offices, public spaces, and residential apartments.  Use cleaning chemicals safely and properly in accordance with department guidelines and assure compliance with local, state, and federal safety standards, MSDS, OSHA, EPA and other relevant rules and regulations. Assist in coordinating the annual deep cleaning program. Schedule the preparation of apartments for move-ins. Conduct inspection of offices, common space, guestrooms, and residential apartments. Maintain inventory control systems for all supplies, chemicals, and equipment. Inform supervisor of any resident issues or concerns. Assist in developing and monitoring the housekeeping department budget. Establish energy awareness in the department. Pay Range- $18-$20/hr

Server- Per Diem- Including Weekends & Holidays

Accept food order, communicate to kitchen and deliver food and beverage to residents and/or guests in a timely manner. Ensure resident/guest satisfaction by responding quickly to needs and requests.  Actively buss and reset tables as well as complete assigned side work. Maintain a positive and professional demeanor toward all residents, visitors and co-workers.

Director of Memory Care

Assess/evaluate potential residents for Memory Care level of care. This may involve assessing/evaluating potential residents in their home or another healthcare setting; must be able to drive to location and perform assessment/evaluation professionally and timely if necessary. Monitor documentation systems to ensure that all regulatory timeframes for review of assessments and service plans are met and that resident needs are met. Write and review residents’ assessments/evaluations and care-plans with appropriate staff. Coordinate, along with the Executive Director and Director of Health and Wellness, family/resident care conferences and serve as a direct liaison between residents, families, and staff. Work with residents, families, and other staff to ensure that residents are at appropriate levels of care. Conduct interviews, orientation, and ongoing training of Memory Care Team employees in accordance with employee training policy and regulatory guidelines.  Coach and counsel departmental employees in accordance with Senior Lifestyle human resources policies.  Manage Memory Care employees in partnership with the Health and Wellness director or designee.  Promote the company’s Embrace philosophy in all aspects of his/her role to include interactions with resident, family, and staff and the daily oversight of Memory Care operations in partnership with the Health and Wellness Director. Organize the monthly programming calendar and needed supplies.  Plan, coordinate, and evaluate Memory Care resident programs. Lead high quality innovative programs in Memory Care to include a minimum of 3-5 self-lead programs daily. Implement and monitor programs and processes to promote excellence in programming while maintaining a high level of resident/family/staff satisfaction in a fiscally responsible manner. Adhere to the budget guidelines for the department. Maintain current knowledge of Alzheimer’s/dementia topics and participates Memory Care Department Director training as assigned. Provide education, training, and hands on support to community staff.

Director of Maintenance

Hire, train, discipline, and terminate department staff. Implement and maintain preventative maintenance programs (TELS and daily checklist). Coordinate compliance with local, state, and federal building codes and guidelines. Coordinate with contractors and venders to make sure insurance and licenses are current. Conduct regular inspections of all Life Safety Systems including fire extinguishers, sprinkler, and alarm systems. Work closely with Executive Director in creating and scheduling capital budgets. Establish and obtain bids for all capital projects.  Coordinate annual maintenance programs for apartments and common areas. Coordinate necessary repairs and maintenance in apartments and common areas including refurbishment for move-ins. Ensure that the outside of the property is maintained including landscaping and snow removal.  Coordinate the removal of garbage and trash for the property. Coordinate department’s staff schedules to ensure the proper maintenance of office public spaces, residential apartments, and grounds. Prepare and follow approved department budget. Maintain inventory control for all general supplies, parts, and equipment. Maintain all required maintenance files. Schedule and conduct training sessions and department meetings. Prepare resident charges/billing for extra services when applicable. Understand and implement the role of the department’s safety and disaster plan.  Head safety committee meetings. Salary Range: $68,640-$74,880

Care Associate – Assisted Living (AL) – Full Time/Part Time

Provide assistance as needed with medication reminders, bathing, grooming, dressing, escort service, and other activities of daily living. Follow the schedule of resident’s needs set out by supervisor. Provide emotional and social support to residents. Document daily log of assistance. Inform supervisor of any resident issues or concerns. Respect and encourage the independence and dignity of the residents. Respect residents’ confidentiality.

Licensed Practical Nurse (LPN) – Assisted Living- Full-Time 2:00-10:00pm

$35.00 per hour Responsible for medication administration for the residents in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines. May be responsible for the admissions of new residents to include all admission assessments and documentation. Responsible for completing acute and long-term care plans or service plans. May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the director. Provide general consultation regarding health-related concerns within the community. Notify physicians and/or family members of any change in resident’s health and provides proper documentation. Respond quickly to all emergencies. Maintain close communication with all departments and staff members. Provide educational meetings for residents and staff. Maintain confidentiality of residents’ and prospective residents’ information.     May be responsible for discharges, assessments, and clinical documentation as needed. 

Sales Specialist – Skilled Nursing

Complete and send out site visit reports every ten days and host the site visit report review with the Divisional Director of Sales and Marketing, Vice President of Operations, and Regional Director of Operations. Schedule, organize, and conduct tours with prospective residents of independent and assisted living, memory care and skilled nursing. Respond and follow-up with all walk-ins, phone inquiries, and mail-ins timely and appropriately and treat each inquiry with value. Meet the targeted sales expectations with respect to call outs, first on-site personal touches, securing deposits, and move ins.  Create, plan, and implement special events at the community to bring in prospective residents and their family members. Take the lead with all sales that become uncertain, and instruct and teach the individual assisting with the lead on immediate next steps and strategies. Participate in huddle calls and all admissions related problem solving sessions.
